Institute sites receive NCQA recognition for diabetes care

The Institute is pleased to share that all 12 of our health centers that offer enhanced diabetes care have received recognition through the National Committee for Quality Assurance (NCQA), a nonprofit organization that assesses the quality and performance of health organizations. The NCQA’s Diabetes Recognition program recognizes clinicians and practices that provide high-quality care to patients with diabetes.
We are committed to providing the best diabetes care to all of our patients. In addition to primary care providers, our team includes Diabetes Care and Education Specialists, case managers, mental health providers, and educational groups and classes to support our patients with diabetes.
