Bronx Health REACH Coalition

BHR Coalition’s mission: To eliminate racial and ethnic disparities in health outcomes in the Bronx by creating a movement of individuals, agencies, organizations, and communities, working together, sharing resources, expertise, information and services.

In order to tackle the disparate burden of chronic disease and related risk factors in the Bronx, more than 80 BHR Coalition members from faith and community based organizations, healthcare, food pantries, schools, small businesses, after school programs, housing and economic groups, and food justice advocates have collaborated for the past 25 years on a community-driven effort to improve population health and reduce health disparities. The BHR Coalition’s membership closely reflects the population in the Bronx, which is predominantly African American/Black and Hispanic.

The Coalition focuses on local determinants of health and cultural considerations to realize health benefits equitably across communities. With empowered residents mobilized, Coalition and community members have sought to improve access and opportunities for healthy living by implementing evidence-based public health activities in nutrition and physical activity, and promoting policy and system level changes.
