Who We Are
Bronx Health REACH (BHR) has been engaging thousands of residents of New York City (NYC)’s the Bronx in a community-driven effort to eliminate entrenched health disparities in chronic disease and related risk factors among African American/Black and Hispanic/Latino residents. These groups represent the majority of Bronx residents, living in the poorest congressional district in the country, who continue to suffer from high prevalence of chronic diseases and other deeply entrenched health disparities. BHR has implemented evidence based and community-informed strategies to improve nutrition, physical activity, and continuity of care in breastfeeding support; as well as increasing demand and access for flu, COVID-19, and other adult vaccines.
The Changing Systems/Changing Communities Model draws on current socio-ecological models of health in general and the People/Place Framework for Health specifically. All have been adapted to fit the unique work of Bronx Health REACH. This model comprises a comprehensive approach to making health equality a reality by recognizing that health disparity cannot be eliminated without eliminating the underlying causes. It also recognizes that it takes a community working together to bring about lasting change.
