Value Based Care

At the Institute for Family Health, we know that a patient-centered, prevention-focused approach is key. We can help individuals and families get the right care at the right time, and avoid more serious illness. We also work with our patients to address the many aspects of their lives that can affect their health, including racism and discrimination, financial security, housing, food and nutrition, safety and more.

Our health centers aim to provide evidence-based and high-quality services that are also cost-effective over the long term. Here are a few of the efforts we’re involved in to promote high-value health care for all in New York:

Along with two other New York State FQHCs, we operate FamilyHealth ACO, a Medicare Accountable Care Organization. Through this initiative, we provide coordinated, high-value health care to Medicare beneficiaries in both New York City and the Hudson Valley.

In the Mid-Hudson Valley, the Institute leads the Ulster County Health Home, known as Care Management Partners of Ulster County, which works to ensure that Medicaid beneficiaries with chronic conditions have access to and coordination of primary and acute health services, behavioral health care and long-term services and support.

The Institute also participates in a Manhattan/Bronx focused Medicaid Health Home known as Community Care Management Partners (CCMP). CCMP is a service model where a diverse network of organizations, including other community health centers, communicate and collaborate with one another so patients’ needs are addressed in a comprehensive, coordinated manner. The Health Home participating organizations work collaboratively to advocate for service and policy changes to improve the health of residents.

The Institute is a member of Community Health Independent Practice Association (CHIPA), New York’s oldest and largest provider association for federally qualified community health centers (FQHCs). CHIPA was founded in 2015 to help New York-based FQHCs align quality improvement strategies, leverage data collection and analytics, and advance value-based care efforts. Through CHIPA, we work to collaborate with insurance plans in a cost-effective and sustainable way.
