Family Health Center of Harlem

This bustling, urban federally qualified health center, located at 119th Street and Madison Avenue in Harlem, provides care to over 20,000 patients of all ages annually. Behavioral health, dental care and more are available onsite.

Walton Family Health Center

A true neighborhood health center, this site offers primary care, behavioral health care and dental care. Walton is also the primary training site for our Family Nurse Practitioner Residency.

Mount Sinai Hospital

The inpatient training site for the Harlem Residency in Family Medicine is Mount Sinai Hospital, located at Madison Avenue and East 98th Street in Manhattan. Founded in 1852, The Mount Sinai Hospital is a 1,141-bed, tertiary-care teaching facility acclaimed internationally for excellence in clinical care.

The Mount Sinai Hospital was named to the U.S. News & World Report Honor Roll in its 2014-2015 edition, ranking 16th out of nearly 5,000 hospitals nationwide.
