COVID vaccines available for ages 6 months and up

The Institute for Family Health continues to encourage everyone in our communities to get the COVID vaccine. We offer the Moderna vaccine for ages 6 months and up.

• If you are a current patient, call 844-IFH-APPT (844-434-2778) to schedule an appointment, or login to MyChart.
• If you are not a current patient, you can schedule online here.

In New York City, appointments are available at Cadman Family Health Center, Family Health Center of Harlem, Walton Family Health Center (Bronx), and Stevenson Family Health Center (Bronx).

In the Mid-Hudson Valley, appointments are available at our Kingston, New Paltz, Ellenville and Hyde Park locations.
