Care Coordination

Managing health care needs and different appointments for yourself or a loved one can be a difficult job. The Institute for Family Health has a team of care coordinators and social workers who can help you navigate the health care system so you can focus on getting well.

Care coordination can involve:

  • Help with scheduling appointments
  • Help with transportation to appointments
  • Getting referrals for the care you need
  • Connection to benefits, insurance or other resources
  • Help with pharmacy and medication home delivery
  • Health education and help with managing chronic conditions

If you or someone you love would like help with care coordination, ask your provider at your next appointment.

The Institute for Family Health accepts Medicaid, Medicare, and most insurance plans. If you do not have health insurance, we can help. Ask us about how to apply for our sliding-fee discount program. You may be eligible based only on income and family size.
