Sliding Fee Discount and Financial Assistance

The Institute’s mission is to provide affordable health care for all who need it. This is why we offer a sliding-fee discount program and other financial assistance. You may be eligible based only on income and family size. No one is turned away.

What is the sliding fee discount?

The sliding fee discount is a discount on services you receive at an Institute for Family Health health center. You may be eligible based only on income and family size. The discount is based on how much money you make. If you make less money, you will pay less for care.

This discount does not apply to services or equipment purchased outside of the Institute such as laboratory testing, medications, and specialty services.

Who can apply?

Anyone can apply for a sliding fee discount. You may be eligible for a sliding fee discount based only on your household income and size. In order to continue using the discount, you must apply every year or when your financial situation changes.

How do I apply?

To apply, log in to your MyChart account. Under Billing, go to Financial Assistance. You can also ask our staff to apply.

We will ask you questions about your income and household size. You must provide documents showing your household income to apply. Here is a list of documents we accept.

What if I’m not eligible for the sliding fee discount?

If you are not eligible for a sliding fee discount and cannot pay the full amount of your care, we can set up a payment plan for you.

What if I’m still worried about the cost of my care?

Ask to speak with a case manager. We will work with you to make sure you get the care that you need and can afford. Don’t skip your appointments or avoid coming in for care because you’re worried about the cost of your care.
