Professional Experience and Accomplishments
Dr. Jacob is the inpatient pharmacy faculty for family medicine residency programs at Mount Sinai Hospital (MSH). She is also faculty for pharmacy residency programs and a clinical coordinator with the Department of Pharmacy at MSH. In addition, Dr. Jacob is an assistant professor at St. John’s University College of Pharmacy and Health Sciences. She is a board-certified pharmacotherapy specialist, board-certified geriatric pharmacist and a certified professional in patient safety. Her practice and research interests include drugs of abuse and misuse, transitions of care, geriatrics, anti-coagulation, patient safety, health equity, inter-professional education and epidemiology. Dr. Jacob is the Founder and President of the Coalition for the Advancement of Pharmacy Practice (CAP), Chair of the Health Equity Committee of the New York City Society of Health System Pharmacists, Chair of the Equity and Inclusion Committee of St. John’s University College of Pharmacy and Health Sciences, and the Secretary/Treasurer of the Geriatrics PRN of the American College of Clinical Pharmacy. Her hobbies include traveling, theater and reading.
