Here to help: care management for chronic conditions

When it comes to living with chronic health conditions, it can be challenging to navigate your care. Balancing appointments, setting up referrals, coordinating refills – it all adds up.

The Institute offers a number of care navigation programs that can help. For example, if you have two or more chronic conditions and are enrolled in Medicare Part B, you may be eligible for chronic care navigation services. Chronic care navigators are individuals who are experts in helping patients get the support they need to better manage their health.

Chronic care navigators can help with:

  • Scheduling appointments
  • Filling out forms
  • Transportation
  • Medication refills
  • Ordering medical supplies
  • Helping with referrals, including to specialists outside the Institute
  • Coordinating with your provider and care team

Your chronic care navigator will reach out to you for brief, regularly scheduled phone calls, with additional follow-ups as needed. Besides coordinating appointments and care, the chronic care navigator can also help you set health goals and work with you to troubleshoot any barriers in reaching those goals.

“What I’ve noticed on my calls with patients is that having someone to guide them really helps with staying on track,” said Lead Care Navigator Nicole Layton. “My profile also shows up in MyChart as a member of the care team. My patients can send me a message anytime and I can coordinate with their providers in MyChart.”
