Your COVID-19 vaccine appointment: Next steps

Thank you for scheduling your appointment for the COVID-19 vaccine. Here’s what you should know about your appointment:

  • It is extremely important that you keep your appointment, or cancel in advance. We cannot waste any doses. If you need to cancel your appointment, call 844-434-2778 as soon as possible.
  • You should not get the COVID-19 vaccine if you have gotten any other vaccines in the last 14 days.
  • Please fill out the NYS COVID-19 vaccine form to confirm your eligibility before your appointment: vaccineform.health.ny.gov. If you don’t have a smartphone or computer, we will help you sign the statement.
  • When you arrive for your appointment, we will ask you to show proof of eligibility such as an ID card. We will also ask you to sign a COVID-19 vaccine consent form.
  • If you have insurance, we will bill your insurance for your vaccine. If you don’t have insurance, it’s ok. No one will pay any out-of-pocket fees for the COVID-19 vaccine.
  • We are offering the Moderna vaccine. The Moderna vaccine is two doses. After you get your first dose, you will need to schedule your second dose 28 days later. You can schedule your second dose through MyChart.
  • Even after you get both doses, it is important to continue to wear a face covering (covering your nose and mouth), practice social distancing, and wash your hands frequently.
  • Please make sure you keep your appointment, so we don’t waste any vaccine. Once we open a bottle of vaccine, we have to use it quickly. If you need to cancel your appointment, please call 844-434-2778.
