Care Management Partners of Ulster County
A New York State Medicaid Health Home Program
The Institute is one of nine agencies who are part of Care Management Partners of Ulster County, a free program that makes sure you get the health care and services you need. Everyone in the program has a care manager who works with you to coordinate everything related to your care, so that you can get on the path to health. We’re here to help, so you don’t have to manage your health alone.
Everyone in the program is connected to a care manager who can help you:

- Connect to health care providers
- Connect to mental health and substance abuse providers
- Access needed medications
- Get help with housing
- Access social services (such as food, benefits, and medical transportation)
- Connect with other community programs that can support and assist you

The program is for adults and kids living in the Hudson Valley who have Medicaid and two or more chronic health conditions. If you have HIV/AIDS or serious mental illness, you are eligible for the program (even if you don’t have a second chronic condition). Children with a serious emotional disturbance or complex trauma are also eligible.
